Ce param\u00e8tre apparemment simple joue un r\u00f4le majeur dans la fiabilit\u00e9 des r\u00e9sultats des tests de champ visuel.\u00b9 C\u2019est aussi un param\u00e8tre cl\u00e9 qui distingue le casque Topcon Inspire\u00ae aliment\u00e9 par RadiusXR\u00ae des appareils concurrents. Contrairement \u00e0 de nombreux concurrents, Inspire respecte les normes cliniques en mati\u00e8re de luminance de fond. L\u2019analyseur de champ Humphrey (HFA), la norme clinique actuelle en SAP, utilise une luminance de fond uniforme de 10 cd\/m\u00b2 (environ 31,5 apostilbs).\u00b9 Le choix de 10 cd\/m\u00b2 n\u2019est pas arbitraire. Elle se situe pr\u00e8s de la limite inf\u00e9rieure de la vision photopique (lumi\u00e8re du jour) o\u00f9 les seuls photor\u00e9cepteurs actifs sont les c\u00f4nes (aucun b\u00e2tonnet n\u2019est actif \u00e0 10 cd\/m\u00b2).\u00b2 En pratique, cela signifie que l\u2019arri\u00e8re-plan est une lueur blanche tamis\u00e9e, pas compl\u00e8tement sombre, pas tr\u00e8s brillante, mais \u00e9quivalente \u00e0 environ 10 chandelles r\u00e9parties sur un m\u00e8tre carr\u00e9. Le test de champ visuel du casque Inspire reproduit pr\u00e9cis\u00e9ment cela, pr\u00e9sentant des cibles sur un fond de 10 cd\/m\u00b2, correspondant intentionnellement aux conditions du Ces valeurs sont sp\u00e9cifiques uniquement aux tests photopiques sur un fond de 10 cd\/m\u00b2.\u00b9 Une luminance de fond standardis\u00e9e garantit que les bases de donn\u00e9es normatives \u2013 ce qui est \u00ab normal \u00bb pour un jeune de 20 ans en sant\u00e9 versus un de 70 ans, etc. \u2013 et les crit\u00e8res de d\u00e9tection des d\u00e9fauts sont comparables entre les appareils. Si un appareil utilise une luminance de fond diff\u00e9rente, ce serait comme comparer des pommes et des oranges, car les sensibilit\u00e9s sont estim\u00e9es sur une \u00e9chelle de mesure diff\u00e9rente. Cependant, tous ces dispositifs ne se valent pas dans la reproduction fid\u00e8le des conditions de test cliniques standard. Beaucoup de VRP testent dans la plage m\u00e9sopique o\u00f9 les c\u00f4nes et les b\u00e2tonnets sont actifs. Par exemple, la Figure 1 montre un exemple de sensibilit\u00e9s ponctuelles pour le VRP Olleyes en utilisant leur algorithme VisuALL-K compar\u00e9 au HFA. La corr\u00e9lation est tr\u00e8s faible (r = 0,11), et la plupart des estimations de sensibilit\u00e9 avec le VisuALL-K sont bien au-dessus de la ligne d\u2019identit\u00e9 (ligne pointill\u00e9e) \u2014 les sensibilit\u00e9s sont syst\u00e9matiquement plus \u00e9lev\u00e9es lors des tests sur une luminance de fond m\u00e9sopique plus fonc\u00e9e.\u2075\u2013\u2077<br\/><br\/>En revanche, le casque Inspire aliment\u00e9 par Radius XR utilise la m\u00eame luminance de fond que le HFA. Une \u00e9tude r\u00e9cente n\u2019a montr\u00e9 aucune diff\u00e9rence statistiquement significative dans les sensibilit\u00e9s ponctuelles entre l\u2019algorithme RATA-Standard de l\u2019Inspire et l\u2019algorithme SITA-Standard du HFA dans la plage de mesure de 15 \u00e0 40 dB de l\u2019Inspire.\u2078 L\u2019Inspire sacrifie la plage de mesure \u2014 r\u00e9sultat de la r\u00e9alisation du VRP dans un casque avec moins de puissance qu\u2019un appareil de table comme le HFA \u2014 pour une forte concordance dans les sensibilit\u00e9s estim\u00e9es. Le r\u00e9sultat est que l\u2019Inspire de RadiusXR peut remplacer ou compl\u00e9ter les p\u00e9rim\u00e8tres traditionnels sans compromettre la qualit\u00e9 des tests. Un appareil qui fait des \u00e9conomies sur ce param\u00e8tre pourrait produire des champs visuels qui corr\u00e8lent mal avec des r\u00e9sultats familiers de Humphrey. De petites \u00e9carts dans les conditions de test peuvent mener \u00e0 des r\u00e9sultats trompeurs. Par exemple, une luminance de fond plus faible pourrait gonfler artificiellement la sensibilit\u00e9, sous-estimant potentiellement le degr\u00e9 de perte de champ ou modifiant l\u2019apparence des d\u00e9fauts.\u00b3,\u2074<br><br>Une autre implication est l\u2019exp\u00e9rience du patient et le flux de travail de la clinique. Comme le casque poss\u00e8de un joint lumineux qui emp\u00eache la lumi\u00e8re ambiante d\u2019entrer, il n\u2019est pas n\u00e9cessaire de tester dans une pi\u00e8ce sombre avec un \u00e9quipement sp\u00e9cial. Les patients peuvent passer le test dans divers contextes, m\u00eame dans une salle d\u2019attente.
